Why a pad that was only flattened sinks under the slab
Flat is not the same as finished. Anybody with a blade can make ground look level. What makes dirt work hold is what you cannot see in the photograph: the soft stuff taken out, good dirt brought in, packed in lifts instead of dumped in one, and the whole thing shaped so water runs off it instead of standing in the middle.
A pad that was only flattened is the one that sinks under the corner of the slab. A driveway that was only spread is the one that turns to soup the first wet February and takes your rock down into the mud with it.

Do you haul the dirt in?
Yes. Sometimes there is enough good material on the property to work with, sometimes it has to come in. He will tell you which one you have when he looks at it.
Do you put the rock down too?
The pad and the drive get built and shaped first — then it is time for new rocks. Tell him what you want on top and he will build the base for it.
Can you put in a culvert?
Yes. Driveway over a new culvert is regular work. If the ditch belongs to the county there may be a call to make first, and he will say so.
